I acknowledge the effectiveness of Acupuncture, Chinese Herbal Medicine, dietary/nutritional therapy, and other TCM methods for preventing and treating various health issues and enhancing overall well-being. While beneficial for many conditions, I understand these treatments do not substitute any medical tests or therapies advised by my physicians. I will maintain my prescribed medications alongside any alternative treatments received at this clinic.
I've shared my health details and medications with the TCM Practitioner/Acupuncturist/Registered Nutritionist and will keep them informed of any changes. The practitioner isn't liable for omissions in information. I recognize no guarantees are given, and the effectiveness of treatments can be influenced by medications and lifestyle. I take full responsibility for my treatment outcomes.
I acknowledge that Acupuncture and related TCM practices are generally safe. Bruising from needle insertion is normal, while fainting is rare but can occur due to extreme nervousness, hunger, or fatigue. Needling sensitive points below the waist or near the breasts may be required, and verbal permission will be sought.
Herbal medicines are usually safe and effective, although some may experience occasional side effects like abdominal upset, diarrhea, or sweating in response to treatment.
Initial 10-minutes consultation is FREE.
The initial Appointment Fee for acupuncture with cupping or TCM Tuina Massage is $135.
The initial Facial Acupuncture Fee is $155 (Tax included) for all patients.
The Initial Bioenergetic Test Fee is $180 +Tax for all patients.
The Appointment fee for food compatibility test, Pregnancy Success Preparation Session and Cancer Recovery Planning Session is $250 +tax each.
The initial consultation fee for Chinese herbal medicine is $80 plus tax. Chinese herbs are available for purchase at cost, with additional charges for shipping and administration. The administration fee for herbal medicine refills is $10 plus tax.
An additional fee of $80 plus tax will apply if you seek nutritional guidance during your acupuncture appointment. You will receive personalized recommendations via text or email, including advice on supplements and foods tailored to your health concerns.
To cover business expenses, a $50 fee will be charged for cancellations or rescheduling with less than 2 business days' notice or missed appointments. This fee is waived in the event of a MEDICAL emergency. It's advisable to reschedule appointments requiring a doctor's visit at least 2 days beforehand to avoid the fee on your next visit.
Please be on time for all your appointment bookings. If you arrive late for your appointment, and you could not receive service due to time constraints, a fee of $50 will apply to your subsequent visit unless it was due to a MEDICAL emergency.
